2005 Transition of Care
You may be changing your medical plan in 2005. If you have an ongoing medical condition and your doctor is not in your new network, you may wish to have help with the transition of your care to a new doctor.

If any of the following conditions apply to you, you may be eligible to remain with your current doctors for a transition period of up to three months:

  • Pregnancy, with expected delivery in the first quarter 2005
  • Extensive therapy such as chemotherapy
  • A terminal illness
  • Waiting for a transplant
  • Behavioral health issues (beyond EAP)
  • Surgery scheduled for January or February 2005

Your new health plan administrator will be able to help you find an appropriate doctor in the network so that your healthcare can be continued without disruption.

You may contact your new health plan administrator via telephone or complete the appropriate transition of care forms. Transition of care forms are provided below.

You must complete and return the Transition of Care form to your new plan administrator in early December to be considered for the transition period. Select the appropriate form from the list below and print it.

BCBS PPO (Medical and Behavioral Health)
Transitional Care Form

HMO Blue New England (Medical and Behavioral Health)
You will need to complete both the
A. Transition of care form
B. Release of Information (ROI) form.

CIGNA (Arizona, North Carolina, Houston, North Texas)
Transition of Care form

Pacific Care
Transition of Care form

Kaiser (Northern & Southern California)
Kaiser does not provide a transition of care form, but encourages the enrollee to either call Member Services or have their physician contact Member Services. (1-800-464-4000)
